Jacobsen Teaching Kitchen and PERx Program

More than 25 years ago, the Turley Family Health Center was founded with a mission to improve access to health care in several medically underserved neighborhoods in Clearwater. Made possible by Linda and Stewart Turley, the Center now teaches the future leaders of medicine, offering comprehensive primary and obstetrical care to more than 7,000 patients annually.

Thanks to the generosity of Morton Plant Mease Health Care Foundation donors, the Jacobsen Teaching Kitchen and PERx Program at Turley Family Health Center are transforming the way chronic disease is managed.

Jacobsen Teaching Kitchen

The Jacobsen Teaching Kitchen is a hands-on learning environment where nutrition meets medicine. Through interactive cooking classes, workshops, and dietitian-led nutrition education, patients gain practical skills to prepare healthy, affordable meals, while family medicine residents learn how to incorporate food as medicine into everyday clinical practice. The teaching kitchen also serves as a hub for community outreach, helping address health disparities, food insecurity, and barriers to healthy living.

PERx Program

The PERx (Personalized Exercise Prescription) Program empowers patients to improve their health through movement. Patients receive individualized exercise prescriptions and musculoskeletal rehabilitation plans designed to increase physical activity, improve function, reduce fall risk, and support self-management of minor injuries and chronic conditions. PERx  helps patients improve energy, mood, sleep, weight, strength, and overall quality of life. In addition to designing exercise prescriptions for general health and rehab programs for patients with musculoskeletal injuries, the Turley Family Health Center's physical therapy assistant runs a Fall Prevention Program for at-risk seniors.

A Foundation for Better Health

Together, these innovative programs at the Turley Family Health Center help to train the next generation of family physicians while providing patients with practical tools to improve their health and well-being. Through culinary medicine, exercise science, and interdisciplinary wellness programs, the Jacobsen Teaching Kitchen and PERx (Personalized Exercise Prescription) Program exemplify the Foundation's commitment to advancing preventive care, improving health outcomes, and creating healthier communities. By investing in prevention, education, and healthy living, Foundation donors are helping patients gain the knowledge, skills, and confidence they need to lead healthier lives for years to come.

The Family Medicine Residency Program at the Turley Family Health Center is helping build the next generation of primary care physicians while expanding access to high-quality, compassionate care for our community. The center provides hands-on clinical education in a collaborative, patient-centered environment where resident physicians care for individuals and families of all ages under the guidance of experienced faculty. In addition to comprehensive primary care, the center offers women's health, sports medicine, behavioral health, clinical pharmacy, lifestyle medicine, nutrition, and a variety of in-office procedures, allowing residents to learn alongside an interdisciplinary team while providing coordinated, whole-person care.
