DESCRIPTION OF 2008 FUNDED PROGRAMS:
Annie Miller Scholarship Over Annie Miller’s 43 years of exemplary service as an RN at Morton Plant Hospital, she earned the well-deserved status as a respected leader, mentor, coach, counselor and friend. Reflecting her commitment to both nursing and education, the Morton Plant Mease Foundation established the Annie Miller Nursing Scholarship Fund. This fund provides scholarships to LPNs who are seeking their BSN degree.
Breast Health Navigator/Educator Provides funding for a full-time Breast Health Navigator who provides initial education for women with newly diagnosed breast cancer throughout the health system. These dollars also help to offset a portion of the screening, diagnostic and treatment services for uninsured women with breast cancer through the Mammography Voucher Program (MVP). The MVP provides educational screenings to women without proper health insurance and financial resources to access breast health services, including mammograms, ultrasounds, additional diagnostic exams, surgical consults, surgeries, and breast cancer treatment. Additionally, this grant supports the Breast Cancer Education Folders with educational material important to any woman diagnosed with breast cancer. Collaborating with community partners, the program provides education and outreach to an average of 1,600 women each year.
Camp Alegria: Empowering Latina Cancer Survivors
The goal of the program is to offer Latinas diagnosed with cancer a positive and memorable experience through a variety of culturally and linguistically relevant educational, social and recreational activities. Objectives are to empower and educate Hispanic cancer survivors about treatment, recovery and overall well-being, as well as, provide a supportive environment for cancer survivors.
Cancer Patient Support Services (CaPSS) When one member of a family has cancer, the whole family is affected. CaPSS was developed in 1991 to address the psychosocial, emotional and spiritual needs of all cancer patients, their families and friends at no cost. The counselors of CaPSS utilize a variety of educational and support services to help cancer survivors and their families deal more effectively with their emotional distress.
Clearwater Free Clinic The volunteer-driven Clearwater Free Clinic provides problem-oriented medical care to the acute and chronically ill low-income, uninsured population living in the Clearwater, Florida area. Morton Plant Mease Foundation funds a grant through Morton Plant Hospital that allows the Clearwater Free Clinic to meet the growing need for additional adult and pediatric appointments, while greatly improving the overall continuity of care and scope of their services.
Comfort Baskets
Provides comfort to families and loved ones standing vigil as a patient enters the final hours of life. Many families do not want to leave the bedside at such a crucial time and therefore do not have access to food, beverages and items that will either help them work through such a difficult time or provide comfort. The Critical Care and Oncology Units at Morton Plant Mease will be able to provide such items for these individuals.
Diabetes Registry Program
Allows for increased patient satisfaction through focused primary care physician attention to the patient's diabetes with enhanced tracking and education. The Registry Program will allow the group to track and improve actual outcomes for the group's diabetic patients.
Dr. George A. Morris, Earn As You Learn Nurse Program The Earn As You Learn program was named in honor of Dr. George Morris, who has devoted his career to advocating nursing quality throughout Morton Plant Mease. This extraordinary program addresses the acute nursing shortage by providing compensation for nursing students to relieve pressures created by the need to work full-time, go to school and provide for their families. This benefits the entire community by contributing to the growth of the nursing programs at St. Petersburg College, PTEC and the University of South Florida. As of December 2008, the Earn as You Learn program will have served 545 students, including four master’s-prepared nurses, 90 RNs, 148 LPNs, and 268 patient care technicians.
Earn As You Learn Nursing Chair This annual $50,000 commitment over a five year period generates funding for additional faculty and recognition for Morton Plant Mease lounge in the new nursing facility.
Education for Diabetic Resource Nurses
The diabetic resource nurses have identified a need for more information on how to educate the diabetic patient regarding their disease management. An adequately educated patient is better able to care for him/herself at home thus decreasing the need for re-hospitalization.
Excellence in Spirituality
The CARE Channel was implemented in all five of our in-patient facilities in 2008. Feedback from patients, families and team members has been overwhelmingly positive. With equipment purchased and installation complete, we simply need to pay our subscription fees in 2009 to continue with this wonderful asset to patient care. Clinical Pastoral Education is the leading international clinical training program for clergy. Based on the action/reflection model of learning, the program provides interfaith, professional education for ministry. The Bereavement Follow-up Program provides support to family members of patients who die in our care, as well as to team members who lose a loved one.
Faith Community Nursing
Allows for the intentional integration of the practice of faith with the practice of nursing so that people can achieve wholeness in, with and through the community of faith. The faith community nurse can be a health educator, facilitator of support groups, counselor and advocate, as well as a spiritual companion to the people in their church. The MPM program utilizes trained Registered Nurses who are currently serving their faith communities or wish to start such an area of service.
Family Practice Residency Program - Turley
Provides physician education and clinical training for residents of the USF College of Medicine. The Residency Program is vital as a primary care physician base and health care services resource for the health system and the underserved community surrounding the Turley Family Health Center. First accredited in 1997, the USF / MPM Family Medicine Residency is an eight resident per year program.
Gestational Diabetes Program
Diabetes education provides women with meter training, medical nutrition therapy and pattern management to promote target glucose values throughout their pregnancy. Physicians refer all pregnant patients for diabetes education, not just the ones who have insurance coverage.
Good Samaritans: Nurse Practitioner Program
Provides funds for a Nurse Practitioner at the Good Samaritans Health Clinic, less than a mile away from Morton Plant North Bay Hospital. Patients coming through the Emergency Room that are not appropriate for emergency services will be directed to the Nurse Practitioner for care. This program will assist in relieving the over crowding of the Emergency Room as well as providing needed support to the under and uninsured patients within the community.
Homeless Emergency Project (HEP): Behavioral Health Team
Since last year, an Emergency Housing Police Outreach Program at HEP has increased demand for Morton Plant Hospital Outreach Services and contributed to the increase in the number of Emergency Room diversions. The MPH Outreach Team is comprised of 2 LPN's and a Psychiatric ARNP who provide services for a population of approximately 400 clients at the Homeless Emergency Project.
Indigent Diabetic After Care - Turley
To date, the attending physicians of Morton Plant Hospital do not have adequate resources to provide uninsured and unassigned diabetic patients referrals for ongoing care and education. This grant will provide patients with consistent follow-up outpatient care post-hospitalization, medication/supply resources and ongoing diabetes education. The goal for patients with little access to the health system outside of the Emergency Room is consistent outpatient access to care.
Indigent Prescription Grant
Allows patient access to a one time only, ten day supply of needed medications on an annual basis. These ten days allow the patient to pursue other, more long term, solutions to their medication access problem.
Katherine T. Smith Scholarship
Provides scholarships for Registered Nurses who are pursuing a Bachelor's or Master's degree in Nursing. We need baccalaureate and masters prepared nurses to serve as clinical experts for staff and to direct care.
La Clinica Guadalupana (LCG): Diabetic Diagnosis & Management
Provides medical non-emergent care to the local indigent population. LCG diagnoses and treats a minimum of 125 diabetics, providing pro-bono physician visits, lab work (through Morton Plant Hospital) and medications to those who have no insurance or government assistance. LCG patients receive a minimum of 30 days' medication and are followed monthly for diabetic services.
Lois Odence Planter's Scholarship
Nursing students have financial needs in excess of those provided by tuition assistance. This scholarship is need-based to help pay for additional expenses such as child care, transportation, etc. Madonna Ptak Alzheimer's Research Center
Provides a range of initiatives including maintenance of ongoing research database, quarterly Care Giver programs, driving evaluations, patient and family transportation, salaries, medications, legal assistance, respite care, clinical research, marketing, initiating new research trials and an outpatient exercise program.
MPM Lung Center
Offers valuable services to the community including smoking cessation, asthma management, support groups and pulmonary rehab.
Palliative Care Services Program A partnership between Hospice and Morton Plant Hospital, which has developed a service focused on meeting the physical, emotional and spiritual needs of hospitalized patients near the end of life. This is an interdisciplinary team composed of a physician, social worker, and chaplain. The project is under the umbrella of the overall Spirituality & Healing Initiative of the health system and Foundation.
Physician Engagement Leadership Academy Established to advance the growth and development of physician leaders. Last year, nearly 60 physicians participated.
Priest Chaplain
At Morton Plant Hospital and Mease Countryside Hospital, we find that patients from the Catholic faith comprise about 20-25% of our daily census. Having a Priest present in the hospital makes an important contribution to the pastoral care and sacramental ministry provided by our health system.
Single Parent Network Helps single parents of the Morton Plant Mease team with emotional and financial support.
S.M.I.L.E. Program Provides stress management tools to the team members of Morton Plant Mease at no cost. Among the many soothing activities offered, team members can receive back or foot massages from licensed massage therapists, enjoy aromatherapy or simply play with pet therapy dogs. All services are free of charge. Twice weekly yoga classes for beginners are also offered as part of the program.
Turley Residency Sports Medicine Fellowship
Two Primary Care Sports Medicine Fellows from the University of South Florida will complete the program each year. The program is projected to provide musculoskeletal specialty services to approximately 500 patients per year. Also, this program will be the only accredited Sports Medicine Fellowship in our geographical service delivery area.
Unfunded Transportation Program Provides transportation for our patients with no resources or funding source to facilitate a timely and safe hospital discharge.
Volunteer Nurse Program Pilot
Volunteer nurses will utilize their professional and clinical skills to supplement the work of staff nurses, providing a valuable one on one time with patients and families. The Volunteer Nurse program provides a pathway for retired nurses, or those taking a break in their career, to stay in their profession and continue their passion for nursing with the flexibility of choosing their own hours and the unit they wish to serve.
Volunteer Week RecognitionPays for luncheon and gift for two hospital auxiliary programs. Wheelchair Transport Van - Rehab
Patients are currently transported to the Morton Plant Rehab Center by Care Ride when discharged from the hospital. The advantage of a new wheelchair transport van is that the patient's discharge time would not be dependent on the Care Ride driver's availability for pick-up, facilitating prompter discharges from MPH.
WOW AwardsThe WOW awards were established in 2004 after the Morton Plant Mease Foundation received an anonymous donation to honor team members with cash awards of $2,500 in recognition of extraordinary service. This gift was given as a tribute to the donor’s physician, Dr. Harry Wilks. The program was named the Wilks Outstanding Winners, better known as the “WOW” awards. The winners of these awards are true role models and an inspiration to the entire health care team. It was the intention of the original donor to inspire others to give additional funds so this program could continue. *Both Morton Plant Mease Foundation and Morton Plant Mease Health Care are 501 (c) 3 organizations.
